Slight blur	A shaky hand, a dirty lens, poor focus up close
Low contrast greyscale	Dim light, glossy paper, a faded print
Inverted	Whether it would also read as light-on-dark

▸What does the quiet-zone check measure?

The blank margin around the code, in modules, estimated from the finder-pattern positions. Four modules is the standard minimum.

▸It decodes at full size but fails when small. What now?

The code has too many modules for the size it will be printed. Shorten the link or print larger.

▸It fails the blur test only.

Marginal contrast or a dense version. Increase contrast first; it is the cheaper fix.
